On March 22, 2026 a compromised privileged signing key minted about 80M unbacked USR and extracted roughly $25M, per Resolv's own post-mortem; the protocol has been paused since, with a recovery portal (KYC required above 10,000 USDC) open through August 26, 2026 that treats pre-incident USR holders as senior creditors compensated near 1:1. Before the incident, transparency ran through Resolv's own collateral-pool dashboard plus an independent Apostro proof-of-reserves dashboard, with most collateral in an on-chain contract wallet and the futures margin in institutional custody; there was no accounting-firm attestor.
What you cannot verify yourself: The recovery administration and its waterfall. The residual token traded near $0.16 on July 1, 2026 on extremely thin liquidity, a print that reflects post-incident residual supply rather than a haircut on pre-incident holders, most of whom are being made whole through redemptions. Historically, the off-exchange futures margin was visible only through Resolv's and Apostro's dashboards.
Hedge leg: The perpetual-futures hedge ran through off-exchange custody at margin wallets that were never publicly disclosed, and is paused post-incident.
